A: Claim forms are available in the office of your school, from Human Resources Services and School Support Centers. Claims forms can also be downloaded from the Standard Life website using the VIP Room from Standard Life and your PIN number.
Q: Life Insurance and LTD do not show on my wallet ID Certificate.
A: These benefits will not show up on your wallet ID certificate because they are provided by different insurance carriers.
Q: How do I submit my dental bill?
A: Your dentist can submit it electronically or you may use the standard claim form provided by your dentist.
Q: How do I add a new baby to my insurance coverage?
A: You need to complete a form to add your new child which is available from Human Resources Services.
Q: How long does my out of country coverage last? And do I need to complete any forms?
A: Coverage lasts for 60 days and you do not need to fill out any forms. In the event of an emergency while out of country, call 1-800-360-6061 for direction.
Q: My son went to school this year and his coverage has terminated. Why?
A: Did you complete a Confirmation of School Attendance? They need to be completed each year and can be picked up at your school. They are to be returned directly to Standard Life.
Q: Where can I get a Standard Life booklet?
A: The booklets are sent out from Human Resources Services
Q: I am going on a Leave of Absence. Can I keep my benefits?
A: Yes but you must pay 100% of the premiums if it is a long term unpaid Leave of Absence.
Q: If I cancel my benefits while I am on my Leave of Absence can they be reinstated?
A: Yes, but note that it is your responsibility to re-enroll in the benefits you want within 31 days of your return to active employment. Contact Human Resources for an appointment to complete the necessary paperwork.
Q: When I retire how long can I keep my benefits?
A: You may continue benefits until the end of the month of your 65th birthday.
Q: I am a teacher. How do I cancel my LTD coverage?
A: Speak to your Federation Representative.
Q: Where do I get my PIN number?
A: Approximately 4-6 weeks after you have enrolled in the extended health plan, you will be sent a wallet ID Drug Card and your PIN number.
Q: What is my certificate number?
A: Your certificate number is a combination of the number ‘1’, your date of birth and the last two digits of your social insurance number, as follows:
For example, if the date of birth is April 30, 1963, and the SIN is 444 555 621, then the new Certificate number is 163043021
Q: How do I find out if a medication or procedure is covered under my extended health plan?
A: Contact Standard Life at 1-800-499-4415. You will need to provide your Plan and Certificate information.
Q: I have lost my drug card. How do I get a replacement?
A: Contact your representative in Human Resources Services at the Education Centre.
Q: I have recently got married (or have entered into a common-law relationship). Can I add my spouse to my benefits plans?
A: You have 31 days from the date of your life status change to add dependents without having to provide evidence of insurability. Human Resources Services can provide you with the forms.
